Visitors to this website are never required to provide contact information in order to read this website, or its psycho-educational blog posts.

Visitors may volunteer name and email address via our contact forms in order to request a clinical appointment, make an inquiry, or request a return phone call.

Under the requirements of the Health Insurance Portability and Accountability Act of 1996 (aka, HIPAA), you will be required to provide personal information when contracting for psychological services. This information is not collected via this website.


While you may be provided with clinical history and financial payment forms as downloadable documents from the website, they cannot be completed on this website, and should not be returned to Suzan Bollich PhD by email. Bring a printed copy with you to your first appointment to best preserve information privacy.


Dr Bollich only has access to information that you voluntarily give via email or phone contact from you. She does not and will not sell, rent or share this information to anyone for marketing purposes, but may be required to surrender the details if required by law enforcement officials and regulations pertaining to mandated reporting.

Dr Bollich will use your information to respond to you, regarding the reason you contacted her psychological services practice. 

Unless you ask her not to, she may contact you via email in the future to tell you about new services, or changes to this privacy policy. Should she send a broadcast emailed newsletter to a list on which you are a member, you may unsubscribe at any time. A link for that purpose will be provided at the bottom of any broadcast emails.
